In addition to the completed claim form, you will need a copy of a valid ID and proof of your Social Security number.
Valid identifiers include:

- Voter registration ID card
- State-issued ID card
- Military ID card
- passport
- Foreigner registration receipt card
Just note that your Social Security number must be located on a computer-generated document with your name and all nine digits displayed, such as your Social Security card, pay stub, or tax form.
